Care1st Health Plan Arizona partners with AHCCCS to support the Whole Person Care Initiative: AHCCCS’ Whole Personal Care Initiative (WPCI), the next system innovation in integrated health care delivery to address social risk factors of health--commonly referred to as Social Determinants of Health (SDOH), which impact a member’s health and well-being. Care1st Health Plan Arizona supports development of the AHCCCS-Approved Statewide Closed-Loop Referral System (CLRS) and promotes provider network and community-based organization (CBO) utilization of the CLRS to properly refer members to CBOs providing services addressing social risk factors of health. Care1st Health Plan Arizona encourages usage of SDOH screening tools available through or compatible with CLRS to screen members for social risk factors of health based upon the provider’s business needs. Regardless of the screening tool selected, it must screen for the following social risk factors of health at a minimum:
- Homelessness/Housing instability
- Transportation Assistance
- Employment Instability
- Justice/Legal Involvement
- Social Isolation/Social Support
