Update to Biomarker Codes Requiring Prior Authorization

Biomarker Codes Requiring Prior Authorization Effective 8/15/2023

Dear AzCH-Complete Care Plan (AzCH-CCP) and Care1st Providers and Staff:

In accordance with recent AHCCCS updates to AMPM 310-KK Biomarkers Testing, effective 8/15/2023 AzCH-CCP and Care1st will require prior authorization for the following codes:

  1. 0163U
  2. 0091U
  3. 81599
  4. 81327
  5. G0327
  6. 83520

Please use the Pre-Auth Check Tools to confirm if a CPT/HCPCS code requires prior authorization.
