This special program is designed to provide support throughout your pregnancy and after you deliver for both you and your baby.

Care Managers help members who need more medical support. They match your needs with medical, social and financial services. Help can be at home, in clinics, hospitals, nursing homes and other healthcare facilities.

Care Managers can:

  • Create a plan to help you manage your condition
  • Find providers and social services to support you
  • Help you understand your benefits
  • Provide education on health issues. These include medications and how to manage them.

Members 6-18 years old get a no-cost Boys & Girls Club basic membership as part of your plan benefits. Show your member ID card to get started.

Your benefits include a three-month membership. Choose a YMCA location near you.

Qualifying members receive vouchers for online WW services.

These services are provided free. For an interpreter for a medical visit, contact Member Services <XX> hours before your visit. If you need something translated into a language other than English, call us. We can also provide other formats, such as Braille, CD or large print.

All services must be medically necessary. Your Primary Care Provider (PCP) will work with you to make sure you get the services you need. These services must be given by your PCP or by another provider that your PCP refers you to.

Some services may:

  • Have copays.
  • Have coverage limits.
  • Need a doctor’s order.
  • Need prior approval.

*Some Medicaid members may not have all the benefits listed.
